The Drop Mums Club: Consent Form
By signing this agreement, you, as the participant, are consenting to:
1. Your child taking part in the mum’s club class.
2. Your child interacting with the entertainment provided.
3. Your child receiving first aid by the instructor if required.
4. The Drop instructors reporting and recording details of any accident suffered by you or your child whilst on the premises.
If for any reason you or your child do not feel comfortable during the class, you should raise this immediately with the instructor. The instructor may ask for your consent to take photographs during the activity and will explain where these photos will be shared or posted. If you do not consent the instructor will not take photographs.
The Drop Mums Club: Policy
This Policy must be read and signed as understood before starting a Mums Club class with The Drop. Failure to complete this form prior to starting will result in the participant being unable to participate in the class.
Scope of Responsibility:
The Drop is a Fitness Studio in Sutton which holds exercise classes and events. The Drop has instructors who are qualified Personal Trainers. Personal Trainers are here to help participants improve their physical health and mental wellbeing through exercise. Please note Personal Trainers are not medical professionals. If anything arises that sits outside of this scope of practice the participant will be advised to seek the help of a medical professional.
Mums Club Overview:
The Drops ‘Mums Club’ classes incorporates gentle exercises to improve strength, mobility, and physical/mental health for mothers from 6 weeks to 12 months postpartum. It can also include weight training. The participant can bring their child aged from 6 weeks up to 12 months to the Mums Club classes with them. It is the instructor’s role to coach and to oversee the participant. The participant is responsible for the health, safety, and wellbeing of their own child.
Health Screening:
We will send you a separate email providing a health screening for all mothers/participant’s returning to exercise. Please go at your own pace during the classes and take it slowly as there is an increased risk of injury to new mothers returning to exercise. Before starting these classes, you must gain approval that it is safe to commence exercise by your doctor- this is normally obtained at your 6 weeks check in. If, for whatever reason, problems with your own health arise during the classes, please stop what you are doing, let the instructor know and then seek the advice of a trained medical professional.
Limitation of Liability
Neither The Drop nor its instructors are liable for harm, personal injury or death arising from participation in any activities during the class or event/workshop unless caused by the negligence of The Drop and/or its instructors.
Health and Safety Instructions for you and your child:
As we are a functioning fitness studio the gym environment can pose a risk to your child’s health and safety.
These instructions must be always adhered to, to help prevent health and safety issues arising:
1. Supervising your child at all times
2. Use the designated spaces for your child to play in. This is detailed as playpens (If your child is crawling or starting to walk), bouncers or mats if they are not crawling or walking.
3. To only let your child out of these areas if you are feeding, holding, or changing them. They must be placed back in their designated areas directly afterwards.
4. To not let your child roam freely around the studio.
5. If performing weight training exercises, to stand a safe distance away from your child.
6. To be aware of other people and children around you. We control the capacity of our classes through our booking system- The Drop | Baby & Toddler Classes Near Me | Happity.
7. We operate on a 1 adult to 1 child ratio. The only exception to this rule may be for a mother with twins however a plan for this will be created separately between the instructor and the participant.
The Drop provides toys, playpens, bouncers, mats, music, and mood lighting within the session for the child’s entertainment. If the participant does not feel comfortable with child’s entertainment or has any other health and safety concerns, they must raise this immediately with The Drops instructors so that the appropriate adjustments can be made.
Health Declaration
‘I, the participant, am declaring that I do not have any known health or medical conditions which exercise could make worse and that in normal circumstances exercise should not harm me in any way. I, the participant, am declaring that my child does not have any known health or medical conditions where participating in the class could make it worse. I shall inform the instructor of any changes to my medical condition or my child’s medical condition prior to commencing the activity. Neither The Drop nor their instructors shall be liable to me or my child for any personal injury which either I or my child may suffer as a result of changes in me or my child’s circumstances of which I was or should have been aware but failed to notify to the instructor and which the instructor would not have become aware by reasonable enquiry. I have received medical clearance from my doctor, nurse or midwife stating that it is safe for me to return to exercise after giving birth and that should any issues arise, I will seek the advice from a trained medical professional.’
By signing this form, you are confirming that:
1. That you have read, understood, and agree with the policy.
2. Neither The Drop nor its instructors are liable for harm, personal injury or death arising from participation in any activities during the class or event/workshop unless caused by the negligence of The Drop and/or its instructors.
3. That you will adhere to the Health and Safety instructions given above.
4. That you will always supervise your child.
5. That you have read and understood the health declaration statement.
